Put the address in square brackets before the port: [2001:db8::1]:8080, or with credentials, user:pass@[2001:db8::1]:8080 or [2001:db8::1]:8080:user:pass. Without the brackets there is no way to tell the address's colons from the port's.
No, that is the usual setup. You connect to a gateway over IPv4 and the gateway sends your request out from an IPv6 address. The checker tests the exit, so these pass as IPv6 as long as the exit is.
The proxy works but could not reach the IPv6-only address, so every request through it leaves over IPv4. For a site that only takes IPv6, or a job that needs a large pool of IPv6 addresses, it will not do.
Connecting to a proxy at an IPv6 address needs an IPv6 route from our checker to that host, and not every network has one. If your provider also offers an IPv4 gateway for the same proxies, check that instead.
